My access to the Questionnaires has expired. What do I do?
By clicking the 'Accept' button on the screen directly before the screen on which you paid by credit card, you let Smithells®NZ know that you had read and understood the conditions listed in the box and accepted the timeline and arrangements. This is the contract you accepted.
The conditions included the period of two months' access only to the Questionnaires.
At this point, you have two options.
If your access to the Questionnaires expired within the last two weeks, you can pay $26.00 for a further two weeks' access to complete the Questionnaires.
Only one extension of access is permitted.
If your access expired more than two weeks ago, you will need to pay $68.00. This gives you a further two months' access to complete the Questionnaires.
